Community Intervention Specialist in Home Support

Part-time | 2 days a week | Saturdays and Sundays | 1-year renewable contract

The Outaouais Housing NPO Federation (FOHO) is recruiting a community intervention specialist in home support for seniors.

Do you want to contribute to the well-being, autonomy, and quality of life of seniors in their living environment? This position will allow you to offer human, personalized, and respectful support, while promoting the autonomy of residents.

Job Conditions

  • Position: Part-time
  • Schedule: 2 days a week, Saturdays and Sundays
  • Contract: 1 year, renewable
  • Start date: As soon as possible
  • Location: Seniors' residence located in Outaouais

Main Responsibilities

Home Support and Accompaniment

  • Accompany residents in their daily activities to promote their autonomy and maintenance in their living environment,
  • Accompany residents for their shopping, essential purchases, appointments, or travel, according to their needs
  • Support residents in certain everyday tasks,
  • Support the organization of daily activities and living habits,
  • Encourage residents to accomplish tasks they are able to perform themselves,
  • Support residents in taking their medication and, when permitted by the applicable framework, ensure the administration of prescribed medication, in accordance with the organization's procedures and required authorizations,
  • Observe changes in a resident's situation and transmit relevant information to the team or concerned professionals

Community Intervention

  • Offer listening, support, and individual accompaniment,
  • Identify situations of isolation, vulnerability, or difficulty,
  • Refer residents to appropriate community, social, and health resources,
  • Support certain procedures related to rights, services, and daily life,
  • Prevent and contribute to the management of conflictual situations,
  • Promote mutual aid, social bonds, and participation in collective life,
  • Organize or support community and social activities,
  • Ensure follow-up notes and transmit relevant information to the person in charge

Profile Sought

  • Diploma in social work, specialized education, community intervention, or related field, or equivalent combination of education and experience
  • Minimum 1 year of experience in community intervention, support relationship, home support, or with a senior clientele
  • Training in medication administration and assistance care, including "Loi 90" type training,
  • Excellent active listening, empathy, and communication skills
  • Autonomy, judgment, initiative, and sense of organization
  • Ability to intervene calmly in difficult situations
  • Ability to work autonomously on weekends
  • Good knowledge of community and social resources, an asset
  • Good command of spoken and written French.
  • Ease with basic computer tools
  • Strict respect for confidentiality

Specific requirements

The person must be able to move around the residence, go up and down stairs as needed, accompany residents during outings or activities, and occasionally transport light equipment.
